OSH compliance has three separate obligations, and only one of them costs money. Every establishment registers, construction projects submit a programme, and certain equipment needs inspection. The penalty for ignoring the standards is measured per day.
The penalty first
Republic Act No. 11058, approved 17 August 2018, sets the exposure.

The daily fine is counted from the date the employer is notified of the violation, or the date the compliance order is served. The amount depends on the frequency or gravity of the violation and the damage caused, and the maximum is imposed only where the violation exposes workers to a risk of death, serious injury or serious illness.
The three acts carrying the separate fine are repeated obstruction or refusal of access to the workplace or records, misrepresentation in a report known to be false in a material respect, and retaliatory measures such as termination, refusal to pay, reducing wages or benefits, or discriminating against a worker who gave information during an inspection.

The system validates completeness before submission, then auto-generates the registration number and a certificate carrying a QR code for verification. The certificate is downloadable from the “My Establishment” dashboard.
Besides the form, you supply a copy of the Business Permit or Mayor’s Permit and an electronic copy of one valid government-issued ID of the employer. DOLE accepts a wide list, including the PhilSys National ID, passport, driver’s licence, UMID, voter’s ID or certification, postal ID, PRC ID, TIN ID, barangay ID or certificate, senior citizen ID, PWD ID and indigenous ID.
Re-registration is required on a change of name, address or ownership, or on reopening after a previous closure.
The construction programme
DOLE Department Order No. 252, Series of 2025 requires every construction project to have a Construction Safety and Health Program, in accordance with RA 11058. It is a set of detailed rules covering the processes, practices and personnel responsible at a specific construction site.

The legal bases DOLE cites are Rule 1970 of the OSHS, a memorandum of agreement between DOLE and DPWH dated 19 July 1983, and Department Order No. 1, s. 1991 as amended by D.O. No. 1-A s. 1991 and D.O. No. 23, s. 1994.
Note the asymmetry on pressure vessels: above 100 cubic feet the inspection marginal rate of ₱8.00 is more than 5 times the plan-checking rate of ₱1.50.
The rights behind the rules
RA 11058 gives workers four rights that the paperwork above exists to serve: the right to know the hazards, the right to refuse unsafe work where DOLE determines an imminent danger exists and corrective action has not been taken, the right to report accidents and hazards, and the right to personal protective equipment free of charge.
Where a work stoppage due to imminent danger results from the employer’s violation or fault, the employer pays the workers their wages during the stoppage.
What this page does not cover
The number and qualification of safety officers, the ratio of health personnel to workers, and the mandatory 8-hour safety and health seminar for all workers are set by DOLE in the implementing rules of RA 11058, which were not accessible on any DOLE host at the time of writing. This page therefore states no worker-count thresholds. It also does not carry the full list of offences and fines that the Secretary issues separately, nor the technical inspection processing times.
