If you are looking for Form 137, the document still exists but the name changed. DepEd renamed it School Form 10 (SF10), the Learner’s Permanent Academic Record, through DepEd Order No. 58, s. 2017.

Why the old name still turns up
The rename was phased by grade level, not applied to everyone at once. This is why two learners in the same school could hold differently named records.

So a record can be substantively correct while carrying either name, depending on when and at what level the learner was enrolled.

DepEd describes SF10 as an individual record of a learner’s academic achievement per level, and DepEd Order No. 11, s. 2018 defines it plainly as the Learner’s Permanent Academic Record, formerly Form 137.
To simplify the format, SF10 bears only the DepEd official logo and seal. The stated reason is consistency and uniformity, to lessen the bulk of stapled forms or the re-writing of the same information onto a school-customised Form 137 whenever a learner transfers.
How it is created
The Grade 1 teacher prepares one copy only of SF10-ES and attaches the relevant supporting documents:
- a copy of the birth certificate;
- the kindergarten completion certificate or Early Childhood Care and Development (ECCD) checklist; and
- the progress report or other equivalent document.
If the learner stays in the same elementary school, the Grade 1 adviser forwards SF10-ES to the Grade 2 adviser, who updates the same form. That repeats through each grade until the learner completes Grade 6.
How a transfer works
This is the part that decides whether a record actually reaches the new school.
- The receiving school requests transfer of the original copy of SF10-ES and other pertinent documents.
- The originating school acts on a written request or a system notification of transfer through the LIS, whichever comes first.
- The originating school creates one certified true photocopy for its own file.
- It sends the original along with the other pertinent documents.
The school you left keeps a copy and sends the original. A school insisting it must keep the original is not following the order.
The LRN
The Learner Reference Number is a unique twelve-digit identification number generated from the Learner Information System and assigned to a learner to track progress through the basic education cycle, regardless of transfer to another school or learning centre in the public or private sector, and regardless of moving up from elementary to secondary level.
It is the number that makes the record findable, which matters more than the form name.

The new forms were mandatory in all public schools from SY 2017-2018, with private schools, SUCs, LUCs and other higher education institutions offering basic education enjoined to adopt them.
