Rome draws its line between spoiled or mutilated and lost. A passport you still hold but have damaged is €60.00, the same as an ordinary renewal, plus €25.00 if it is mutilated. A passport you cannot produce is €150.00 plus €25.00, or €175.00 all in, which is 2.92 times the routine fee.
On validity the Embassy states that citizens 18 years and above are issued 10-year passports and minors 5-year ones, that applicants 13 years and above should sign page 3 of the booklet immediately on receipt, and that minors 12 and below should leave the signature line blank.

Neither page carries a date. The Schedule of Fees states €10.00 in three separate rows, for certification per copy, for notarial and certification services, and for visa applications, so it is internally consistent; the €11.00 appears once, on a single service page. That is a reason to treat €10.00 as the better-supported figure, but it is not a statement from the Embassy, so both are printed.
The €60.00 against €66.00 gap matters more, because €60.00 is also what Milan charges to solemnise a marriage. Rome’s schedule agrees with Milan and Rome’s own marriage page does not.
