PhilHealth registration is one form and one number. The form is the PMRF; the number is your PIN, and you only ever get one.

The premium is 5.0% of monthly income effective January 2025, between a ₱10,000.00 floor and a ₱100,000.00 ceiling, so ₱500.00 to ₱5,000.00 a month. The contribution table sets out every band.

PhilHealth Circular No. 2026-0011 covers self-paying members: those who personally pay their own premiums.

The legal footing is RA 11223, the Universal Health Care Act. Section 5 on Population Coverage provides that all Filipino citizens shall automatically be included in the National Health Insurance Program.
