Quick answer

A PSA death certificate costs ₱130.00 as a viewable online copy, paid at a PSA CRS outlet and viewable for 60 days. PSA needs the complete name of the deceased, the date and place of death, the place of marriage, and the requesting party's name, address, copies and purpose.

Where these figures come from

About these figures: the numbers below come from PSA's official published sources: Philippine Statistics Authority | Death Certificate (https://psa.gov.ph/death-certificate, retrieved 2026-08-31); PSA Serbilis (https://www.psaserbilis.com.ph/, retrieved 2026-08-31); Philippine Statistics Authority | Census Serbilis Center Directory, Metro Manila (https://psa.gov.ph/directory/census-serbilis-center-metro-manila, retrieved 2026-08-31). Agencies change fees and rates without notice, so confirm with PSA before you rely on them. Last verified: 2026-08-31.

A PSA death certificate is ₱130.00 as a viewable online copy. The information PSA asks for is slightly different from a birth or marriage request, and that difference is where most failed searches come from.

What PSA needs to find the record

PSA publishes the exact list for a death certificate:

  • complete name of the deceased person
  • date of death
  • place of death
  • place of marriage
  • complete name and address of the requesting party
  • number of copies needed
  • purpose of the certification

The place of marriage line is the one people miss. It sits on PSA’s own list for a death request, alongside the place of death.

Who may request one

PSA Serbilis publishes the entitlement rule for civil registry documents generally.

# Who may request
1 The owner themselves, or a duly authorised representative
2 Their spouse, parent, direct descendants, guardian or institution legally in charge, if a minor
3 The court or proper public official, whenever absolutely necessary in administrative, judicial or other proceedings to determine identity
4 In case of the person’s death, the nearest of kin

Row 4 is the operative one for a death certificate.

If someone else collects it

# What you submit
1 Your signed authorisation letter or Special Power of Attorney, stating the requested documents, the number of copies and the purpose
2 Your valid ID with photo and signature, including the National ID
3 Your representative’s valid ID with photo and signature

PSA Serbilis notes that in certain cases additional proof of relationship must be presented on release, such as an authorisation letter, Special Power of Attorney or court order. The National ID guide covers the ID named explicitly there.

The three request routes

Route Where What it needs
Walk-in Census Serbilis Centers An appointment booked at appointment.psa.gov.ph
Online, for delivery psaserbilis.com.ph or psahelpline.ph Delivered anywhere in the Philippines or abroad
Post or email By conventional post The certification is returned by conventional postal service

If PSA has no record

PSA issues a Negative Certification where no record is found. PSA Serbilis publishes an Electronic Endorsement Request Form for that, free of charge: fill it out online, print it, and present it to the Local Civil Registry Office together with the Negative Certification.

What this page does not cover

PSA does not publish, on the pages read for this guide, the price of a printed and delivered death certificate, delivery timeframes, the DocPrint fee, the appointment mechanics at appointment.psa.gov.ph, or the process for registering a death that was never recorded. None of those are stated here.

The PSA birth certificate guide and the PSA marriage certificate guide cover the other two certificates at ₱130.00. The PSA online request guide covers ordering.

Frequently asked questions

How much is a PSA death certificate?
PSA Serbilis publishes ₱130.00 for a viewable online copy of a birth, marriage or death certificate, paid at any PSA CRS outlet. A CENODEATH, the Certificate of No Death Record, is ₱185.00. PSA does not publish the printed and delivered price on the pages read for this guide, so this page does not state one.
What information does PSA need?
The complete name of the deceased person, the date of death, the place of death, the place of marriage, the complete name and address of the requesting party, the number of copies needed and the purpose of the certification.
Why does PSA ask for the place of marriage?
It is on PSA's own published list for a death certificate request. PSA does not explain why on that page, so no explanation is offered here beyond noting that it helps verification.
Who may request one?
PSA Serbilis publishes the general entitlement rule for civil registry documents: the owner or a duly authorised representative; their spouse, parent, direct descendants, guardian or institution legally in charge if a minor; the court or a proper public official where absolutely necessary in proceedings to determine identity; and in case of the person's death, the nearest of kin.
How long is the online copy good for?
60 days, through an access code. Within that window you can have a security-paper copy printed via the DocPrint service at a PSA CRS outlet.
What if PSA has no record of the death?
PSA issues a Negative Certification and may advise you to go through the Local Civil Registrar. PSA Serbilis publishes an Electronic Endorsement Request Form for that, free of charge, which you print and present to the Local Civil Registry Office with the Negative Certification.
What is a CENODEATH?
The Certificate of No Death Record. PSA Serbilis lists it alongside the CENOMAR at ₱185.00 for a viewable online copy, against ₱130.00 for a death certificate.
Who delivers it?
PSA Serbilis states its deliveries are handled only by LBC Express within the Philippines and DHL Express internationally.
