Check the encoding before you sign
The Embassy’s warning is worth repeating in full. Your photo and biometrics are captured at the encoding area, and your signature on the passport enrolment certificate indicates consent to the details as encoded for printing.

Applicants based overseas may also use a host-government-issued ID showing Philippine citizenship, such as a residence card. All IDs must be readable, untampered and consistent with the documents presented, and originals plus photocopies are required at application.

A lost-passport file needing a birth certificate and a marriage contract carries ₱260.00 in PSA fees before the Embassy’s own charge, and the PSA online copy lapses at 60 days, so order it close to the appointment.
