A lost passport at KWD 48.750 is exactly 2.5 times the KD 19.500 renewal, and that is before the affidavit at KWD 8.125.

The name-change documents are a Certificate of Marriage where a married woman will use her spouse’s surname; an annotated Certificate of Live Birth where entries were corrected; an annotated Certificate of Marriage showing annulment, divorce or court ordered instruction; or a Certificate of Death of Spouse where a married woman reverts to her maiden name.
A woman who opts to retain her maiden name needs no PSA Certificate of Marriage, the same rule Kuala Lumpur publishes.

Check it before you sign
The Embassy states plainly that once signed, any errors in the passport are no longer the fault of the Embassy, and the replacement fee is shouldered by the applicant. At KD 19.500 a re-application is not trivial.
The legal basis it cites is Republic Act No. 8239, the Philippine Passport Act of 1996, under which acquiring a passport is described as a privilege while the applicant’s constitutional right to travel is recognised.

Note the timing clash: a PSA document is valid 60 days, and the passport wait is up to 45 days. Order the PSA document after your appointment, not before, or it may lapse while you wait.
