Kuala Lumpur publishes an unusually precise checklist, and two of its rules turn on dates printed on your ID rather than on anything about your passport.
What the Embassy does not publish
A fee. The pages read for this guide carry no passport charge, so this page states none. Confirm the amount with the Embassy before you travel to an appointment.

The name-change documents are a Certificate of Marriage, an annotated Certificate of Live Birth, an annotated Certificate of Marriage showing annulment, divorce or a court ordered instruction, or a Certificate of Death of Spouse.
If a woman opts to retain her maiden name, no PSA Certificate of Marriage is required.

Those two cut-offs are the trap. A Postal ID from before November 2016, or a seaman’s book from before February 2020, is not accepted here even though both are perfectly valid documents elsewhere, PNP accepts any unexpired Postal ID for a police clearance, with no issue-date condition at all.
The Voter’s Certificate detail
A COMELEC Voter’s Certificate is accepted, but only one issued from the COMELEC main office in Intramuros, Manila. A certificate from a local office does not meet the Embassy’s wording. The voter registration guide covers the certification service.

Refusing a National ID without just and sufficient cause carries a fine of ₱500,000.00 under RA 11055, which is why the PhilID heads the Embassy’s own list. The National ID guide sets that out.

- Is a seaman's book accepted as ID?
- Yes, if issued from February 2020 onwards. The Embassy names the Seafarer's Record Book and the Seafarers Identity Document issued by MARINA, with that date cut-off.
- Can I use the digital National ID on my phone?
- A printed copy must be submitted. The Embassy also states you will be required to generate the digital ID via national-id.gov.ph or the eGovPH mobile application during processing, for document verification.
- Do I need a PSA marriage certificate?
- Only if your name has changed. The Embassy asks for PSA documents supporting a change of name, and states that a woman who opts to retain her maiden name does not need a PSA Certificate of Marriage.
